- How do I get my Doctor’s recommendation?
You need to come into our office for a history and physical examination by our physician.
- What do I need to bring with me?
You should bring the following, concerning your chief medical complaints for which you desire treatment from medical cannabis:
A. A valid Californian government issued ID, driver’s license, or passport;
B. Copies of your previous medical records, physician’s statements, disability records or reports, or prescriptions lists
C. Copies of X-rays, MRI’s, Utra-Sound, or CT scans, other imaging films and /or reports,
laboratory tests results or reports,
D. Medication bottles or medication lists
- What if I don’t have my medical records or reports?
You should download a copy of the “Authorization for Release of Medical Records” form to present to your primary or specialty physician for record copies or sign a release in our office (do bring your doctor’s name, address, and telephone number).

- How much is the evaluation/examination?
Our fee for the evaluation and your recommendation is
$150 cash or credit
card/ debit card (M/C or Visa). We do not accept AMEX or Discover.
- How do I make an appointment?
The Claremont office requires no appointments. We do evaluations on a walk-in basis. The Newport Beach office has hours by appointments only and
Saturday/Sunday hours are available for your convenience.
- What are your hours?
The Claremont office is open Monday and Wednesday for 1pm to 6pm and Friday 12pm to 5pm. Newport Beach is open by appointment only on Saturday or Sunday.
- Where are you located?
The Claremont office is located at 678 S. Indian Hill Blvd., Suite 302, Claremont, Ca 91711 on the third floor of a brick and brown professional building, 1 block north of the 10 Freeway on the north east corner of Indian Hill Blvd. and San Jose Ave. ( across the street from BC Café).
